Home Based Support Service (HBSS) Facilitator

Job Description

The HBSS Facilitator is responsible for the provision and documentation of services in accordance with all guidelines from the Department of Human Services.

ESSENTIAL FUNCTIONS

Essential Functions Statement(s)

  • Advocate on behalf of client and family.
  • Establish rapport with guardians and/or family members.
  • Responsible for completion of field notes, reviews, ISP's, ICAP's and consents.
  • Must possess knowledge of entitlements and be a resource to clients, families and other agencies.
  • Must be a liaison for IDT.
  • Must complete home visits or vocational visits bi-monthly.
  • Must complete intakes for the department.
  • Must present at HRC/BMC, if client has restrictions.
  • Must provide financial guidance to client and monitoring for assigned program.
  • Responsible for the generation and collection of program data.
  • Convenes the community support team as required to revise the Individual Plan as part of the interdisciplinary process.
  • Informs the direct supervisor of all conditions that may prove detrimental to the clients well being.
  • Conducts all phases of client service in compliance with the laws on confidentiality and HIPAA.
  • Assess client's skills and abilities using preferred tools.
  • Must utilize fiscal agents.
  • Regular, reliable, predictable attendance.
  • Performs all other duties as assigned by Supervisor.

SKILLS & ABILITIES

Education:

Bachelor's Degree (four year college or technical school): Required

Experience:

1 plus years of experience in IDD or MI related field

Computer Skills:

Evolv, Microsoft Office, Word, Excel, Powerpoint, Outlook

Certifications & Licenses:

Qualified Intellectual Disabilities Professional (QIDP) Certification or eligible to become QIDP following the Department of Human Services standards.
